New Patient Intake Forms - Required for all new patients or for patients returning to our office after an extended absence (over two years). 

You may complete it on your computer then print it out, or you can print it then fill it in manually.  Please bring the forms with you to your appointment. 

 

Doctor's Referral Form - Doctors can complete the form on their computer, print, sign & mail to our office.  As always we appreciate the confidence you have in our practice!

CitiHealth Card Application - This is our premier financing option.  Fill in the search criteria, our ZIP code is 12553.  Next, select 'Windsor Dental Center' then click continue.  Follow the prompts to continue completing the application.  Please review the terms and conditions of the application prior to completing it.  Keep in mind you will need your estimated treatment cost to complete the application.  If you have any questions, please do not hesitate to contact our office.
